For Women
HOUSING SUPPORT FOR FEMALE INCARCERATION SURVIVORS
We aim to work with female inmates before release to identify a path to short-term and long-term housing. Beyond support with housing searches in Hennepin and Ramsey Counties, placement, and maintenance, we aim to offer a holistic approach to serving people to help them maintain their housing stability. This approach includes focus areas in holistic healing, harm reduction, community and family reintegration, and skill and employment development and support.
Services We Provide

One-on-One intake to develop individualized Reentry Plan. Plans will outline immediate housing, moving/trasition support, resource connections, benefit screening, employment exploration

Short-term housing placement in a shared setting in the community​

Support in transitioning to long-term housing solution through housing search, housing move-in, and support to maintain housing​​​

A 2nd Program staff will provide holistic support to the women to promote successful reintegration, healing, and long-term stability

We have experts in trauma-informed care, life coaching, case management, housing stabilization, and workforce development

Facilitation of individual and group therapy

Building strong community relationships to address systemic challenges and barriers

Beyond Housing

Supporting life skill development including job readiness skills, financial literacy training, and social skill workshops​

Partnering with employers in the community to assist in securing job opportunities​

Housing Stabilization Services billed through Medical Assistance​

Referrals to other physical and mental health care providers as necessary
Quality Assurance
01.
Individual Reintegration Plan review every 30 days to make adjustments to personal goals, recognition of accomplishment and to set new goals to keep individuals on track

Data Review and Outcome Check every 30 days to track goal targets and achievements as well as adjust areas that are underperforming
03.
Participant Survey upon exit or every 6 months. Survey will be used to assess program effectiveness and make improvements
